Here is what you must know about the disability screening camp organised by administration

Two-day disability screening camp is going to be held at the Sonabarighat Moinul Hoque Choudhury H.S. School, under the guidance of hon’ble deputy speaker Aminul Hoque Laskar and district administration Cachar. The event is been organised by National Institute of LocoMotor Disability, Kolkata in collaboration with SAKSHAM, South Assam and supported by many local NGOs.

Facilities to be provided after screening: –

Hearing instruments for Deaf.

Wheelchair for Locomotor, Paralisis, Cerebral Palsy, Divyangs Artificial Limbs for Locomotor, Divyangs Smart Cane for Visually Impaired persons, TLM kit for mentally retarded persons.

Also club foot and hand operation will be provided pperation facility after screening under ADIP Scheme and correction surgery for leprosy deformity cases.

The hearing impaired persons who can be cured by Cochlear implants may be referred for surgery under ADIP scheme (5-12yrs).

Following documents will have to be in camp:

1. Two copies of photographs (showing disability)

2. Photocopy of residential proof (Voter I card/ Ration card/ Aadhaar card). For minor, father’s Voter ID & child’s birth certificate.

3. Photocopy of Disability Certificate

4. Family income certificate from competent authority (MP/ MLA/ DM/ ADM/ SDO/BDO/ Councilor/Gram Pradhan)

Please ensure that the beneficiaries has not received aides and appliances from any sources in the last three years in case of beneficiaries above 12 years and last one year in case of beneficiaries below 12 years.
